An RFI number, project and date, who’s asking and who owns the answer, one clear question, the drawing or spec reference, a suggested solution, any cost/schedule impact and a need-by date. The template has all of these.

One question per RFI, always reference the drawing, propose an answer, set a need-by date, and track status in a single log rather than in email threads.

Usually the architect or engineer of record. The "ball-in-court" field on the template makes it explicit who currently owns the answer at any moment.
